To provide transparent information to Survivor Advocates about a project or engagement opportunity, please complete this project brief.  The brief aims to inform survivor advocates and assist in their choice to participate in the project, with consideration to risks and benefits of the engagement.

The Best Practice Engagement with Survivor Advocates Guide has been developed to assist you to plan, deliver and reflect on your engagement with survivor advocates.  Please read the Guide before submitting this form.

Can privacy and confidentiality of the Survivor Advocate be protected?
i.e.., use of first name only, use of pseudonym, not using name or images in promotional materials, not disclosing location details
Will information be recorded and shared?
